Concerns
Field/Option | Description |

As evidenced by | Optional. Enter the symptoms or indications to support the concern. For example, if the patient’s concern is insomnia, they may report difficulty falling asleep, even after taking over the counter sleep aids. |
Associate to active diagnoses | All concerns must be associated with at least one active diagnosis. Begin typing the diagnosis to associate with a concern and then select it from the menu. Alternatively, click the down arrow in this field to select from all diagnoses. You can associate multiple diagnoses with a concern. Note: The active diagnoses menu not only lists diagnoses added during a patient encounter, but lists diagnoses from all patient encounters. Diagnoses added directly to the patient’s Problems list will not appear in the menu. |
Concern/Presenting problem |
Enter a patient concern. Begin typing a word or phrase that best describes the concern to address. Select a matching concern from the menu. Alternatively, click the down arrow in the field to select an existing problem as the concern. |
Delete Treatment Plan |
Permanently deletes a treatment plan. When prompted, click Submit to confirm the deletion. |

Optional. After entering the patient's goals, begin typing the name of a goal or click the down arrow in the menu to view all goals. Select one or multiple goals to associate with a concern. |
