Field/Option Description
Apply Templates

Click to access a library of predefined goals you can add to a plan. In the Apply Templates search field, begin typing the name of a template to locate it. Alternatively, click the down arrow in the search field to browse all templates. Select one or more templates to apply. In the templates select the goals to include.

Baseline description

Optional. Enter additional information about the goal if needed.

Goal priority

Optional. Select from None, Low, Medium, or High. When you save the goal, the priority level appears in the upper right corner. If you select None for a goal, a No Priority status appears.

Goal statement

Optional. Enter the goal phrased in the patient's own words. You do not need to enter quotations in this field.
Note: When entering a patient's goal statement, there's no need to include quotation marks. Quotation marks automatically appear around the patient's statement when you save.

Goal title Use to manually enter a goal. Enter a statement summarizing the goal (up to 300 characters) or select a suggested title from a dropdown library of athena-curated content.

Session type

Optional. Select the type of therapy the patient is receiving from you to address the goal.

Start date, Target date, and Review date

Optional. Enter the date the patient intends to start working on the goal. In Target date and Review date, select a length of time the patient finds reasonable to achieve or check the progress of a goal.

Targeted concerns

Optional. Begin typing the concern to link to the goal and select it from the list below. Only the concerns you’ve added for the active treatment plan appear in the menu for selection. Alternatively, click the down arrow in the field to browse all concerns. Select one or more concerns to associate a goal.

Treatment modalities Optional. Indicate the types of therapy the patient is engaging in to meet their goal. This can include treatment modalities the patient is engaged in outside of therapy sessions, such as art or animal assisted therapies.
