Attest and Send Requests

After selecting the patient's SMS and/or email address, and selecting supervisor providers (if applicable), click to submit the attestation artifact for approval.

Attest to Treatment Plan

In the attestation artifact, review the PDF copy of the treatment plan your patient will receive. Review the legal statement beneath the PDF viewer. Your Full Name and the current Date appear in read-only fields under this statement.

Cancel

Exit out of an attestation artifact without sending it.

Send Treatment Plan Attestation Requests

Select the contact methods available — via SMS and/or via Email. Check for a mobile number or email address. Only one contact method is needed.
Note: If the patient has no contact information, click Cancel. You must have at least one patient mobile number or email to complete attestation. See Patient contact method issues to troubleshoot.

To providers If applicable, select a supervising provider to include in the attestation process. The provider you assign will receive the treatment plan to review. Select more than one provider if needed.
