Care Plan Goal Sets

On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Exam, click Care Plan Goal Sets

To add or edit goals and instructions in a patient's care plan, you must be able to update the Assessment / Plan section of an encounter or order group.

1. Display the Care Plan Goal Sets page: On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Exam, click Care Plan Goal Sets.
2. Name — Enter a name for the goal set.
3. Provider — Select the providers who can use this goal set.
4. Ordering — Enter a value to set the display order of this goal set.

5. Add Goal — Enter at least two characters to search for the finding to add, and then select it from the list.
   Note: Available care plan goals are derived from the athenaOne global lists of elements for vitals, findings for social history questions, findings for HPI and PE templates, and lab analytes. (Physical therapy goals are based on HPI and PE templates for physical therapy.)
6. Target Value — Enter a target value.
7. Time Frame — Select a time frame for the goal.
8. Primary — For physical therapy goal sets, select a primary goal to automatically populate G-codes in the billing tab.
9. Repeat step 5 through step 8 for each goal in the set.
10. Other Goals — You can use this field to add an unstructured goal to the set.
11. Click Add.

Note: You can edit, copy, or delete goal sets by clicking the corresponding link next to each goal set.
