Past Medical History Questions
This page allows you to administer the past medical history questions that appear in the History section of the patient chart. You can add questions and control the order of the questions in the section. You can also delete or update existing questions.
Caution: Use extreme caution when editing checklists and medical history questions. You should edit field names only to fix typos or edit abbreviations. Making substantial changes to content or changing the sequence of checklist items and questions affects all patient charts that include the checklist or question.

On the Main
Menu, click Settings > Clinicals. In the left menu, under Practice Links — History/Intake, click Past Medical History Questions

To access this page, you must have the Clinicals Admin: Past Medical History Questions role or permission.


- Display the Past Medical History Questions page: On the Main
Menu, click Settings
> Clinicals. In the left menu, under Practice Links — History/Intake, click Past Medical History Questions.
- Click Add new.
- QUESTION — Enter a question.
- Ordering — Enter a digit in the field to indicate where in the list the question should appear.
- Click Save or Save and Add Another.

Caution: Use extreme caution when editing medical history questions. Edit a past history question only to fix typos or edit abbreviations. Making changes to question content or changing the sequence of questions affects all patient charts that include the question and may retroactively create documentation errors.
- Display the Past Medical History Questions page: On the Main
Menu, click Settings
> Clinicals. In the left menu, under Practice Links — History/Intake, click Past Medical History Questions.
- Click the edit link next to the medical history question that you want to update.
The item details appear in the fields at the top of the page.
Important: If you need to change the meaning of a question, create a new question on this page (see To create a past history question). - Edit the fields as desired.
- Click Save.
The updates appear in the list below.

- In the patient chart, click the History tab. At the top of the History section, click the Go to menu
and select Past Medical History.
- If clinical staff answered questions, these questions appear with their answers in this list below the Past Medical History heading.
- To view all questions, including questions that have not been answered for the patient, click the view more icon
.
- Select Yes or No to answer unanswered questions or to edit answered questions. Click the add note icon
to add a note.
- Click the view less icon
to close the item details.

- On the Main Menu, click Settings
> Clinicals.
- In the left menu, under PRACTICE LINKS — History/Intake, click Past Medical History Questions. Locate the "Show in Patient Self Check-In" column.
- When adding a new question, the default answer to "Show in Patient Self Check-in" is "Yes."
- When editing an existing question with a null value (neither Yes or No) in the Show in Patient Self Check-in column, the default answer to "Show in Patient Self Check-in" is also "Yes."

This scenario describes a situation where you should not edit a past medical history question.
- Dr. Joe Smith receives an updated guideline for patients with diabetes that suggests a history of foot surgery requires an earlier foot exam.
- Dr. Smith opens the Past Medical History Questions page to update the relevant question based on the new guidelines.
- Dr. Smith finds an existing question called "Diabetic Foot Exam" and clicks edit.
- After reading the warning message, Dr. Smith clicks Cancel to leave the existing "Diabetic Foot Exam" question unchanged.
- Dr. Smith clicks Add new to create a new question called "Foot surgery."
By creating the new question, Dr. Smith avoids changing the previously documented answers to the "Diabetic Foot Exam" question. The new question is now built in to the encounter for all providers in the practice.
