If a serum creatinine lab analyte value is available, the glomerular filtration rate (GFR) value is calculated when the flowsheet opens. If you manually enter a serum creatinine value in the flowsheet, the GFR is not calculated. You cannot edit this value.
GFR equation for patients 18 years and older
athenaOne calculates the GFR of patients 18 years and older using the MDRD-4 equation:
GFR (mL/min/1.73 m2) = 175 × (Scr)-1.154 × (Age)-0.203 × (0.742 if female) × (1.212 if African American)
Note: If the patient's race is not available, the GFR is not calculated.
Serum creatinine (Scr) must be in mg/dL. This equation is the MDRD-4 revised equation and includes modifications from the original MDRD (Modification of Diet in Renal Disease) study. This equation to calculate GFR is supported by the National Kidney Foundation.
Note: The NIH recommends that, given the limitations of the MDRD equation, any GFR value that exceeds 60 be considered simply as "≥60 mL/min/1.73 m2," not as an exact number. In athenaOne, we display numerical values that exceed 60 because these values may be valuable for practices that track GFR trends over time. However, we caution you to interpret these numbers in light of the NIH recommendations.
GFR equation for patients younger than 18 years
athenaOne calculates the GFR of patients under 18 years of age using the Bedside Schwartz equation:
GFR (mL/min/1.73 m2) = (0.41 × Height in cm)/Creatinine in mg/dL
Note: A recent length/height is required for this calculation. For patients under 1 year old, a length recorded in the last 30 days is required. For patients older than 1 year but younger than 18 years of age, a length/height recorded in the last 90 days is required.
