Document Stage for athenaClinicals
Document is an alternative third stage of the 5-stage patient encounter that replaces the Exam stage for One-Stage Focused Visits and One-Stage Focused PT Visits.
When you begin an encounter for an appointment that is assigned to the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout, you see only a few key sections in the Document step of the encounter to help you get through the encounter more quickly.
To document encounters that use the One-Stage Focused Visit or One-Stage Focused PT Visit, you must have the Clinicals permission.
To manage the mapping of appointment types to encounter layouts on the Assign Appointment Types to Layouts page, you must have the Clinicals Admin: Chart/Encounter Layout permission.
To manage appointment types on the Appointment Types page, you must have the Schedule Admin: Appointment Cancel Reasons/Types role.
Before you can use the Document stage in a One-Stage Focused Visit, you need to decide which types of appointments you want to use these encounters in and assign those appointment types to the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout.
- Display the Assign Appointment Types to Layouts page: On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Chart/Encounter, click Assign Appointment Types to Layouts.
- Layout — Select an encounter layout from the menu.
- Specialty — Select a specialty.
- Assign appointment types to encounter layout — Select the appointment types that should use the selected layout.
- Click Add.
Note: Changes to appointment type–encounter layout mappings apply only to encounters that have not yet been started.
If you prefer to create a new type of appointment in which to use the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout, you can create a new Appointment Type.
- Display the Appointment Types page: On the Main Menu, click Settings > Schedule. In the left menu, under Practice Links — Scheduling, click Appointment Types.
- Click Add new.
Additional fields appear. - Name — Enter a name for the appointment type.
- Short name — Enter a unique short name that is four characters or fewer. This short name appears in the Today's Appointments worklist and in the schedule.
Note: Choose this name carefully: after you schedule an appointment of this type, you cannot edit the Short name. - Duration — Enter the duration of the appointment type, in minutes.
- Appointment type class — Select the appointment type class from the menu.
- Schedule color — Click Select to choose a background color for the appointment type.
- Ordering — Enter a number to indicate the order of the appointment type. If blank, the list is ordered alphabetically.
- Notes — Enter any notes for this appointment type. These notes appear on the confirmation page after you schedule an appointment.

- Patient time —
- Select Yes if this appointment type is reserved for time with patients.
- If you select No, you cannot book a patient into a slot of this type, and the rest of the fields in this section are disabled. You can select No for appointment types such as Meeting or Lunch.
- Generic slot —
- Select Yes to include this appointment type when searching for other non-restricted appointment types.
- Select No to exclude this appointment type when searching for other non-restricted appointment types.
- Restrict search results —
- Select Yes to exclude generic appointment type slots when searching for this appointment type.
- Select No to include generic appointment type slots when searching for this appointment type.
- Template only —
- Select Yes if this appointment type should appear only on scheduling templates. If you select Yes, users cannot schedule appointments of this type.
- Select No if users should be able to schedule appointments of this type.
- Billing slip required —
- Select Yes to have the Missing Slips worklist display appointments of this type.
- If you select No, these appointments do not appear in your Missing Slips worklist.
- Patient time —
- Patient Portal (Appears only if your practice uses athenaCommunicator)
- Display name — Enter the name for this appointment type as it should appear on the Patient Portal.
- Click Save.
- Begin an encounter for an appointment type that uses the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout.
- In the Document step, document the encounter using the standard sections.
athenaOne may add sections to the Document step if you select:- A reason for visit that's associated with an encounter plan that includes additional sections
- The Newly referred patient, patient being referred back or new patient check box in the Reason for Visit section (When this box is selected, the Documents for Discussion section appears. Once added, this section cannot be removed, even if you clear the check box.)
- If additional sections are needed, click the add icon next to the appropriate sections from the Add Sections menu.
Prerequisites: The appointment must use the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout, and the section cannot contain any data.
- Hover over the section in the Document step of the encounter.
- Click X
Note: You cannot remove the following sections:- Reason for Visit
- Assessment & Plan
- Documents for Discussion
- Begin an encounter for an appointment type that uses the One-Stage Focused Visit or One-Stage PT Focused Visit encounter layout.
- Click the arrow under the Document step in the patient banner.
athenaOne lists the standard sections and any sections you added to the encounter. - Select the section you want to go to.
For short appointments — such as an injection, sick visit, or post-operation visit — you can document the encounter on a single page that includes only the sections you need.

Note: The only difference between the One-Stage Focused Visit and the One-Stage PT Focused Visit encounter layouts is that the One-Stage PT Focused Visit includes two additional sections by default: Physical Therapy Episodes and Exercise Flowsheets.

- Post-op
- Injections
- Workers compensation
- Vaccines
- Flu shots
- Lab-only visits
- Physical therapy evaluations
- Occupational therapy evaluations
- Behavioral health evaluations
- Care management follow-ups
- Nurse visits

Note: athenaOne adds the Documents for Discussion section when you select the Newly referred patient, patient being referred back or new patient option in the Reason for Visit section. After this section is added, you cannot remove it.
These sections are unavailable in the Document stage of the encounter; however, you can access most of these sections elsewhere.

From an encounter plan
If the reason for visit that you select is associated with an encounter plan that includes additional sections, athenaOne automatically adds those sections to the Document step.
Documents for Discussions section
If you check Newly referred patient, patient being referred back or new patient, athenaOne automatically adds the Documents for Discussion section.
Note: After the Documents for Discussion section is added, you cannot remove it, even if you clear the check box.
Add sections
You can add other sections as you need them. Just click the add icon next to the appropriate sections from the Add Sections menu.
Reusing sections from a previous encounter
You can also add additional sections by reusing a section of a previous encounter. Just go to the Visits tab, click the view more icon next to an encounter, and then click the reuse arrow next to the section you want to reuse.
Remove sections without data
To remove a section, hover over the section and click X.
Important: You can remove only sections that don't contain any data. If you attempt to remove a section that contains data, you see a message letting you know you cannot remove the section.
Exceptions: You cannot remove the following sections:
