User Guide — Automated Care Pathways
athenahealth Population Health
Automated care pathways create care plans and automatically add assessments and goals to the plan. If a care manager enrolls a patient in a program or adds a focus problem to the patient's care plan, the assessments and goals associated with the program or focus problem are automatically added to the patient's care plan.
Your organization must use athenahealth Population Health, and you must have a role that includes one of these permissions to access this feature:

Before you can use automated care pathways, you must contact your Customer Success Manager (CSM) to assist you with setup.
Automated care pathways create care plans that are focused on a patient's needs, conditions, and problems and automatically add assessments and goals to the plan. athenahealth Population Health provides two types of care pathways:
- Program-based — Patients are identified, referred, and enrolled in specific care management programs. If a patient is enrolled in a program configured for automation, all assessments and goals are automatically added under the General Wellness focus problem.
- Focus problem-based — Focus problems allow care managers to identify, document, and prioritize patient problems. If a focus problem configured for automation is added to the patient's care plan, the assessments and goals associated with the focus problem are automatically added to the patient's care plan.
In both types of care pathways, assessments and goals can be added to the workflow. In addition, assessments and goals can be associated with newly created programs or focus problems to create an automated care pathway.
Example
As a care manager, you work with your organization and your athenahealth CSM to set up an automated care plan for a diabetes focus problem for your type 2 diabetes patients. The discussion and resulting care pathway might resemble the figure below. With the configured automated care plan in place, athenahealth automatically recommends care tasks for diabetes patients when you add diabetes as a focus problem.
Before you can use automated care pathways, you must work with your athenahealth Customer Success Manager (CSM) to create the care pathways.
