PCP Center — Severity Tab

The PCP Center Severity tab displays the Severity Detail page — severity and risk information for a PCP's patient population. This tab is available to Milliman clients. A primary care provider (PCP) may be a physician, practice, or group.

 

On the Main Menu, click Reports. Under POPULATION MANAGEMENT, click PCP List. Click a PCP name in the list. In the PCP Center, click the Severity tab.

 

 

 

Your organization must use athenahealth Population Health, and you must have the Population Management: PCP Center Severity Tab permission to access the Severity tab.

 

 

 

If you use Milliman, the Severity tab displays a severity stratification dashboard for the primary care physician's panel.

 

 

Across the top of the page, these risk scores appear for the PCP's patient population:

  • Total Risk Score
  • Rx Risk
  • IP Risk
  • OP Risk
  • Emergency Risk
  • Other Risk

You can click a risk score to see details about the Top Groups, Top Providers, and Top Members that contribute to the risk score on the right of the page.

 

 

 

On the left, you can stratify the severity data by Clinical Class, Payor, Age, or Gender.

  • If you click Clinical Class,
    • You can click one of the Clinical Classes that appear (such as Migraine) to view a list of members in that class.
    • You can also view more by clicking View the Complete List at the bottom.
  • If you click Payor, Age, or Gender,
    • You can click a payer, age, or gender that displays to view a list of members. For example, if you stratify by Gender, you can click Male to view members who are male.
    • At the bottom of this list you can choose to Compare Severity for the information displayed.

 

 

On the right, you can click Top Groups, Top Providers, or Top Members to view more details about risk scores and Total Adj. PMPM for the selected Clinical Class, Payor, Age, or Gender, display.

  • Click a member's name to go to their Person Center.
  • At the bottom of this list you can choose to Compare Severity.

Click Print to print information, as desired.

 

 

 

 

 

You can filter most PCP Center tabs by month and year. The END DATE filter lets you select both a month and a year, so you can see how utilization and severity data change from month to month. The Efficiency tab also has a Begin Date filter.

  1. On the Main Menu, click Reports. Under POPULATION MANAGEMENT, click PCP List. Click a PCP name in the list.
  2. Update the END DATE filter:
    1. Click END DATE near the top of the page.
      The END DATE box appears.
    2. Select the year by clicking on << or >>
    3. Click the month in the year of interest.
    4. Click Apply.
