Person Center — Medications Tab

Use the Person Center Medications tab to keep track of a patient's current and historical medications.
On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Meds tab on the left side.
Your organization must use athenahealth Population Health, and you must have the Population Management: Person Center: Meds (Edit) permission to edit the contents of the Meds tab. If you have the Population Management: Person Center permission, you can view but not edit the Meds tab.
Medications are tracked both manually and automatically, providing you with a more complete picture of patient's current and historical medications.
You can manually add, edit, and reconcile (or unreconcile) information about a patient's medications.
Population Health automatically adds information about a patient's medications based on claims data received via payer data feeds, as well as EMR data feeds.
Population Health receives information about a patient's medications from payer data feeds and EMR data feeds. New data is automatically grouped when it matches an existing medication on either the patient's Active or Historical list.
If the data does not match one of the patient's existing medications, a number appears in orange next to Reconciliation to indicate how many medications remain unreconciled. The number of unreconciled medications also appears in the patient briefing.
You can control how claims data is reconciled and organized on the Meds tab:
- Review unreconciled data and move the data to a patient's Active, Historical, or Rejected list.
- Unreconcile data as needed.
- Group similar medications together to help organize and make lists more manageable.
- Display the patient's Person Center Medications tab: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Meds tab on the left side.
- Click the plus icon at the upper right.
The "Add a medication" window appears. - Medication — Begin typing the name of the medication, and then select the desired medication and dosage strength from the list.
Additional fields appear. Only the Medication field is required. - Take — Enter the amount of medication and select a unit of measure.
- Every — Enter the frequency.
Note: When selecting the frequency of a medication, you can enter a numeral and then select an option, such as hour[s], from the list. However, if you select "twice a day" or "3 times a day" from the list, the numeral field is disabled, and any numeral entered for frequency is automatically cleared. - Start date — Click the calendar icon to select a start date.
- Click Active or Historical.
- For historical medications, you can also complete these fields:
- Stop date — Click the calendar icon to select the date when the patient stopped taking this medication.
- Reason — Enter the reason why the patient stopped taking this medication.
- Click Save.
The medication appears in the patient's Active or Historical list of medications.
- Display the patient's Person Center Medications tab: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Meds tab on the left side.
- Click the name of a medication in the Active or Historical list.
The area expands to show editable fields. - Modify the amount, frequency, start date, stop date, or reason as needed.
- Click Save.
- Display the patient's Person Center Medications tab: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Meds tab on the left side.
- To move a medication to the Historical list and specify a stop date and reason:
Click the name of the medication in the Active list.
The area expands to show editable fields.
Click Stop Medication.
- Stop date — Click the calendar icon to select a stop date.
- Reason — Enter the reason for stopping the medication.
Click Save.
The medication moves to the patient's Historical list of medications, and the number of medications next to HISTORICAL increases by 1.
- To move a medication to the Historical list without specifying a stop date and reason:
- Point to the name of the medication in the Active list.
- Click the X icon .
Note: You can edit the medication in the Historical list later to specify a stop date and reason.
- Display the patient's Person Center Medications tab: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Meds tab on the left side.
- Click HISTORICAL.
A list of the patient's inactive medications appears. - Point to the medication and click the Reactivate icon in the upper right corner.
The medication moves to the Active list, and the number of medications next to Active increases by 1.
- Display the patient's Person Center.
- Click the Meds tab.
A list of the patient's active medications appears.
Note: You can click Historical near the top of the tab to view the patient's historical medications. - To view a brief description of each grouped item, click [#] Grouped Items.
- To view detailed information about each grouped item:
- Point to a parent medication.
- Click the Flowsheet icon to view all medications that are grouped together.
Each medication in the group appears in bold with a brief summary. - To view more information, click a medication summary.
Details appear in a table.
On the Meds tab of a patient's Person Center, click:
- Claim to view filled dates for medications as indicated by claims data
- EMR to view filled dates for medications as indicated by EMR data
- Manual to view the dates of manually entered data
- Flowsheet icon to view all medications that are grouped together; click a medication summary to view all related data
- Display a patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Meds tab.
- Click Reconciliation to view details about unreconciled data (indicated by an orange number).
- Filter By: — You can select a data source to filter the Reconciliation list:
- ALL — Show data from all data sources.
- MANUAL — Show only data that was entered manually.
- CLAIMS — Show only claims data that was received from a payer data feed.
- EMR — Show only data from electronic medical record (EMR) feeds.
Note: Data received from EMR feeds is labeled EMR.
- For each item in the Reconciliation list, you can move the item to the list you are currently viewing (Active, Historical, or Rejected) using one of these methods:
Note: To move an item to a list you are not currently viewing, you must first click Active, Historical, or Rejected near the top of the tab to display the desired list.- Click the Move icon .
- Drag an item from the Reconciliation list to the "Drag and drop unreconciled items here" area of the Active, Historical, or Rejected list.
- Drag an item from the Reconciliation list and drop it on top of another medication to move the item and group it with an existing medication in one step.
The number of grouped items appears as a link under the medication.- To view grouped items, click [#] Grouped Items.
- To hide grouped items, click Hide Grouped Items.
- To add documentation for a grouping, click the group, or the parent item, to view editable fields. You can make changes only to parent medications; you cannot edit other items in a grouping.
- You can then manually group active, historical, and rejected medications together to better organize each list. (See Step 3: Group similar medications together for more information.)
- Display the patient's Person Center.
- Click the Meds tab.
- Click the Flowsheet icon next to a medication.
Note: The new data alert appears only if there is new data for the medication. - Click a medication summary.
Note: When you group similar medications together, each medication appears in bold with a brief summary. - Click a medication to view details in a table.
- Review any new data (indicated in orange).
Note: New data is automatically grouped if it matches an existing medication on the patient's Active, Historical, or Rejected list. - Click Mark all data reviewed to acknowledge this new data.
Note: To move a medication back to the Reconciliation list, click UNRECONCILE.
- Display the patient's Person Center Medications tab.
- To move a medication to the Historical list and specify a stop date and reason:
- Click a medication in the Active list.
The area expands to show editable fields. - Click Stop Medication.
Fields related to stopping the medication appear. - Stop date — Select the date to stop this medication.
- Reason — Enter the reason for stopping this medication.
- Click Save.
- Click a medication in the Active list.
- To move a medication to the Historical list without specifying a stop date and reason:
- Point to a medication in the Active list.
- Click the X icon .
Note: You can edit the medication in the Historical list later to specify a stop date and reason.
- On the Meds tab of a patient's Person Center, click HISTORICAL.
- Point to a medication in the list.
- Click the Reactivate icon .
The medication moves to the Active list.
- Display a patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Meds tab.
Any relevant notes appear below each medication.
- Display a patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Meds tab.
- Click the plus icon .
- Complete the required fields for the medication.
- Click the note icon .
- Enter notes about the medication.
- Click Save.
- Display a patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Meds tab.
- Click a medication.
- If there is no existing note, click the Note icon .
- Enter notes about the medication.
- Click Save.
- Display a patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Meds tab.
- Click a medication.
- Update the notes about the medication.
- Click Save.
- Display the patient's Active, Historical, or Rejected list on the Meds tab.
- Click Reconciliation.
- Drag and drop a medication onto another medication to group the two together.
- Repeat these steps to group all similar medications.
See To view grouped medications for information about how to see all medications in a group and to view details about each medication in table view. - To ungroup items, click the Flowsheet icon ; click a medication summary to view more details; and click UNRECONCILE.

You can click a medication in a patient's list of active or historical medications to edit its attributes.
- Medication — Shows the name of the medication.
- Take — You can change the amount of medication and select a unit of measure.
- Every — You can change the frequency.
