Patient Request Notes
This page allows you to administer patient requests related to HIPAA privacy regulations. You can use this page to create and track patient privacy requests for:
- Record amendment
- Copy of record
- Accounting of disclosures
- Other
On the Privacy Information page, scroll down to the Privacy Notes: Other Requests section and click the link for the appropriate request (record amendment, copy of record, accounting of disclosures, or other), or click update next to an existing request.
- Display the patient's Privacy
Information page: On the Quickview, click Manage Privacy (under the Privacy heading).
At the bottom of the Privacy Information page under the Privacy Notes heading, the Other Requests field has a link for each type of request that affects the patient's record. Any existing requests are displayed in a list. Click update to view or edit a request. - To add a record amendment request, click the record amendment link. The Patient Request Notes page appears.
- Type — Verify or select the type of privacy note that you want to create.
- Source — Select the source of the request.
- Form — Select the appropriate form (if applicable) from the Form list. (Your practice must have at least one form or document defined to use this list.)
- Request Date — Enter the date that the request was made (usually the current date).
- Review Date — If you are authorized to adjudicate patient privacy requests, set the date that you reviewed the request.
- Status — Verify that the status of the request is set to REVIEW. (If you are authorized to adjudicate patient privacy requests, you can determine and set the appropriate status at this time.)
- Request Note — Enter a description of the request. It is essential to fully describe what restrictions are being requested.
- Review Note — Enter a description of the review and decisions made.
- Click Save. The patient Quickview page appears, with the number of privacy notes in REVIEW status updated. The new request also appears on the Privacy Information page under the Privacy Notes heading.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading).
- From the Privacy Information page, click the update link (under Privacy Notes: Other Requests section). The Patient Request Notes page appears.
- Locate the request that you want to update in the Patient Request Notes list. (You can use the filter fields to limit the list.)
- Click the update link. The privacy note details appear in the fields at the top of the page.
- Update the fields as needed. (If you are authorized to adjudicate patient privacy requests, you can determine and set the appropriate status at this time.) When you follow up on a request, it is essential to enter the review date, the new status of the request, and notes describing actions taken and the reasons for them.
- Click Save. The patient Quickview page appears, with the number of privacy notes in REVIEW status updated. The updated request note also appears on the Privacy Information page under the Privacy Notes section.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading).
- On the Privacy Information page, click the update link (under Privacy Notes: Other Requests heading). The Patient Request Notes page appears.
- If there are many notes listed, you can use the filter fields under the Patient Request Notes heading to locate the privacy note that you want to delete.
- Click the delete link. A warning message appears.
- Click OK to delete or Cancel to preserve. If you click OK, the patient Quickview page appears. The request note no longer appears on the Privacy Information page, but it appears as disabled in the list of patient request notes. An undelete link replaces the delete link. The audit history of the note remains available.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading).
- From the Privacy Information page, click the update link (under the Privacy Notes: Other Requests heading). The Patient Request Notes page appears.
- If there are many notes listed, you can use the filter fields under the Patient Request Notes heading to locate the privacy note you want.
- Click the update link. The patient request note details appear in the fields at the top of the page.
- Click the show history link. The audit history of the patient request note appears.
