Care Plan Report
The Care Plan Report allows you to oversee, prioritize, and track the progress of care management activities for a patient or panel of patients. You can use insights from this report to:
- Help ensure that patients with care plans are receiving routine care.
- Display up-to-date data on care plan utilization.
- Reduce time identifying when care management services can be billed.
Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library. Click the Clinicals tab. In the Standard Reports section of the tab, click run next to Care Plan Report.
To access reports on the Clinicals tab of the Report Library, you must have the Clinicals user permission and the Report: Report Library: Clinicals permission. The Report: Report Library: Clinicals permission is included in the following roles:
- Practice Superuser role
- Report Reader role
Note: To activate the Clinical Inbox Audit report, use the Activate Reports page.
- Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library.
- Click the Clinicals tab.
- Click run next to Care Plan Report in the Standard Reports section of the tab.
The Run Report: Care Plan Report page appears. - Report Format — Select the report format:
- HTML table — Display the report results on your screen.
- Text (tab-delimited) — Export the report results to a .csv file in tab-delimited format.
- Text (comma-delimited) — Export the report results to a .csv file in comma-delimited format.
- Report Options — Select the desired options:
- Suppress Column Headings — Select this option to remove column headings from the report results.
- Suppress Report Name — Select this option to remove the report name from the report results.
- Run Offline (will appear in your Report Inbox tomorrow morning) — Select this option for very long reports. Reports that are run offline appear in your Report Inbox the morning after the request.
- Click Run Report.

EnterpriseID | The patient's athenaOne enterpriseID that represents the entire patient record across all provider groups. |
ChartID | The identifier for the chart the care plan is associated. |
Patient Name | Patient's first and last name. |
Patient DOB | Patient's date of birth. |
Phone Number | Patient's preferred phone number. |
Care Manager(s) | Name of the designated care manager(s). |
Primary Insurance |
Patient's primary insurance reporting category. |
Department Name | Patient's current active department. |
Usual Provider |
Name of the patient's usual provider. |
Last Appointment Date | Patient's last scheduled appointment. |
Next Appointment Date | Patient's next upcoming scheduled appointment. |
HCC RAF Score |
Generated by the athenaOne risk engine. |
Care Plan Created | Date the active care plan was created. |
Care Plan Last Updated | Date of the latest update to the care plan (could be care plan creation or updates to Care Team Task, Patient Task, Goal, or Reason for Care Plan) |
Care Plan Last Updated By | The athenaOne username of the last person who updated the Care Plan (could be care plan creation or updates to care team task, patient task, goal, or reason for care plan). |
Last CM Event Date | Date of most recent care management event. |
Last CM event Added By | The athenaOne username of who added the most recent care management event. |
Total Minutes Current Month | Relative to when the report was downloaded. The current month is when the report was last downloaded. |
Total Minutes Previous Month | The month before the report was last downloaded |
Enrolled in CCM | Is the patient enrolled in CCM? (Yes or No), determined in Quickview. |
Enrolled in Episodic |
Is the patient enrolled in episodic care? (Yes or No), determined in the chart Care tab. |
Enrolled in Longitudinal |
Is the patient enrolled in longitudinal care? (Yes or No), determined in the chart Care Management tab. |
