Quick Reference — To create a Patient Portal account without an invitation email (family members)

A family member who is not a patient at your practice can create a Patient Portal (or Patient Information Center) account on behalf of a family member who is a patient at your practice. For example, a father can create a Patient Portal account for his minor child, so that he can access her health information.

Note: If your practice does not allow non-patients to create Patient Portal accounts, this feature is not available to your patients.

 

If the age of majority for your practice's Patient Portal is 18 years old, a family member can register to access their 19-year-old child's Patient Portal account, but the family member will have billing-only access (instead of the full access they would have if the child was 15 years old).

 

Instructions for patient's family member

  1. On the Patient Portal home page, click Sign up today.
  2. Who will use this account? — Click the Patient's family member option.

Enter information about the patient

  1. First name — Enter the patient's first name.
  2. Last name — Enter the patient's last name.
  3. Date of birth — Select the patient's date of birth.
  4. Gender — Select the patient's gender.
  5. Email — Enter the patient's email address.
  6. Phone — Enter the patient's phone number.
  7. Is this a mobile phone? — Select Yes or No.
  8. I'm not a robot — Check the box, enter the CAPTCHA information, and click Verify.
  9. Click Continue.

Verify your identity

  1. The Patient Portal will send a temporary passcode to the patient (not to you).
    Note: By sending the temporary passcode to the patient, we inform the patient of your request to obtain access to his or her Patient Portal account. By giving you the temporary passcode, the patient consents to your receiving access.
  2. Select how this passcode should be sent: email, call, or text message (these options depend on the patient contact information that is on file with the practice).
  3. Enter the email address or phone number and then click Send Code.
  4. Enter the temporary passcode that the patient received by email, phone call, or text, and then click Continue.

Enter your demographic information

  1. Relationship to patient — Select your relationship to the patient.
  2. First name — Enter your first name.
  3. Last name — Enter your last name.
  4. Email — Enter your email address.
  5. Date of birth — Select your date of birth.
  6. Phone — Enter your phone number.
  7. Is this a mobile phone number? — Select Yes or No.
  8. Click Continue.

Set your password

  1. Password — Enter a password that meets the criteria on the right. This password must be 8-20 characters long and must have at least one uppercase letter, one lowercase letter, and one number or symbol.
  2. Confirm password — Enter your password again.
  3. Remember this computer to save time resetting your password — If you're using your own computer or a computer that you trust, you can select this option to make it faster to reset your password if you ever need to. If you're using a public or shared computer, do not select this option.
  4. Click Terms and Conditions and Privacy Policy to read both documents, and then check the box to accept the Terms and Conditions and the Privacy Policy.
  5. Click Continue.

You are now logged in to your own Patient Portal account, with third-party access to your family member's account.
