Chart Printing Form Configuration
This page allows you to administer the set of default forms that render the way information is displayed on system-generated printed charts.
On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Forms and Letters, click Chart Printing Forms
Note: You can use the Clinical Paper Forms page to create custom forms for each chart-printing element.
- Display the Chart Printing Form Configuration page: On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Forms and Letters, click Chart Printing Forms.
- Using the menus provided, select a form for each element in the printed chart. To select the system default form, select the blank option from the menu.
- Cover Sheet: Identifies the document as a medical record document
- Facesheet: Displays a summary view of the patient's medical information
- Encounters: Displays a summary of selected encounters
- Prescriptions: Displays a summary of orders for medications, including renewals
- Click Save.
In athenaOne, the "formal heath record" is the selection of documents and parameters that should be used when generating a patient's official medical record. Your practice is provided with a default configuration for formal health records. This format can be edited in athenaOne at any point via the Formal Health Record section of the Chart Printing Form Configuration page.
- Display the Chart Printing Form Configuration page: On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — Forms and Letters, click Chart Printing Forms.
- Under the Formal Health Record heading, review the documents listed under the Selected Attachments box on the right.
- Use the arrows to make any desired changes to the selection.
- Click Save.
The menus on the Chart Printing Form Configuration page allow you to specify a form to use when printing each element in the printed chart. The menus contain the forms available in your practice. A form may contain a letterhead, HTML code, magic words, and so forth.
You can print the patient chart using the Chart Export page.
Note: Only clinical paper forms that do not have a document class assigned appear in the menus.
Any custom cover sheet should contain a statement regarding medical record confidentiality that complies with all applicable laws and regulations. Your custom cover sheet must include one of the following:
- Phone number: 1-888-482-8436.
- Message: "This fax may contain legally privileged health information and is intended for the sole use of the intended recipient. You are hereby notified that the disclosure or other unlawful use of this health information is prohibited."
You can add the phone number or the message manually, or you can add them using one of the WRONGNUMBERMESSAGE magic words (see Clinical Paper Forms). If your custom cover letter does not include either the phone number or the message, we add our default cover sheet to the fax.
Sample statement
This fax may contain sensitive and confidential personal health information that is being sent for the sole use of the intended recipient. Unintended recipients are directed to securely destroy any materials received. You are hereby notified that the unauthorized disclosure or other unlawful use of this fax or any personal health information is prohibited.
If you received this fax in error, please visit www.athenahealth.com/NotMyFax to notify the sender and confirm that the information will be destroyed. If you do not have Internet access, please call 1-888-482-8436 to notify the sender and confirm that the information will be destroyed. Thank you for your attention and cooperation.
