Patient Disclosure Notes
This page allows you to view, log, update, and delete disclosures of patient protected health information. You can also view the audit history of patient information disclosures.
Federal regulations (HIPAA) give patients the right to request and receive an accounting of disclosures of their protected health information for reasons other than treatment, payment, healthcare operations, or those specifically authorized by the patient or government.
On the Patient Actions Bar, click Registration, and then click Privacy Information. On the Privacy Information page, click disclosure (under the Privacy Notes section: Disclosures)
On the Quickview, click Manage Privacy (under the Privacy heading). On the Privacy Information page, click disclosure (under the Privacy Notes section: Disclosures)
On the Main Menu, click Patients. Under CHART MANAGEMENT, click Privacy Notes List. On the Privacy Notes List page, click Generate Worklist. The Privacy Notes Worklist appears in the Task Bar. Click a linked PATIENT NAME in the worklist to display this page in the Workspace.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading).
- Click the disclosure link (under the Privacy Notes section: Disclosures) to display the Patient Disclosure Notes page. The Patient Disclosure Notes page appears.
- Source — Select the source of the request.
- Form — Select the appropriate form (if applicable). Your practice must have at least one form or document defined to use this list.
- Request Date — Verify, enter, or select the disclosure request date. This is usually the current date.
- Review Date — If you are authorized to review requests, enter or select the date that you reviewed this request. This is usually the current date.
- Status — Verify that the status of the disclosure request is set to REVIEW. (If you are authorized to adjudicate patient privacy requests, you can determine and set the appropriate status at this time.)
- Request Note — Enter the description of the disclosure request. Describe the nature of the disclosure request.
- Review Note — If you are authorized to review requests, enter any notes about reviewing this request.
- Click Save. The Quickview page appears, with the number of privacy notes in REVIEW status updated. The disclosure request also appears on the Privacy Information page under the Privacy Notes heading.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading). The Privacy Information page appears.
- Click the update link for the disclosure note that you want to update. The Patient Disclosure Notes page appears with the disclosure request details in the fields at the top of the page.
- Update the fields as
needed. (If you are authorized to adjudicate patient privacy requests,
you can determine and set the appropriate status at this time.)
When you follow up on a disclosure request, it is essential to enter the review date, the new status of the request, and notes describing actions taken, including what information is to be disclosed, to whom, and for what reason. - Click Save. The Quickview page appears, with the number of privacy notes in REVIEW status updated. The updated disclosure note also appears on the Privacy Information page under the Privacy Notes: Disclosures section.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading). The Privacy Information page appears.
- Click the update link for the disclosure note that you want to update. The Patient Disclosure Notes page appears with the disclosure request details in the fields at the top of the page.
- Click the delete link. A warning message appears.
- Click OK to delete or Cancel to preserve. An undelete link replaces the delete link. The audit history of the disclosure note remains available.
- Display the Privacy Information page: On the Quickview, click Manage Privacy (under the Privacy heading). The Privacy Information page appears.
- Click the update link for the disclosure note that you want to update. The Patient Disclosure Notes page appears with the disclosure request details in the fields at the top of the page.
- Click the show history link. The audit history of the disclosure note appears.
