Hospital Visit History Report

The Hospital Visit History report displays all visits for all levels of care during a selected date range. This report filters visits based on the end date of the bill for that level of care. If the visit is still open, the end date of the bill for a visit defaults to the current date.

 

Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library. On the Other tab, click run next to the Hospital Visit History Report report in the Standard Reports section.

 

 

 

To access reports on the Other tab of the Report Library, you must have the Report: Report Library: Other permission. This permission is included in the following roles:

  • Practice Superuser role
  • Report Reader role

Note: To activate the Hospital Visit History report, use the Activate Reports page.

 

 

  1. Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library. On the Other tab, click run next to the Hospital Visit History Report report in the Standard Reports section.

  2. Date Range — Enter the start and end dates for the report, or select a date range from the menu.
  3. Report Format — Select the format for your report results.
    • HTML table — Display the report results on your screen.
    • Text (tab-delimited) — Export the report results to a .csv file in tab-delimited format.
    • Text (comma-delimited) — Export the report results to a .csv file in comma-delimited format.
  4. Report Options — Select report options.
    • Suppress Column Headings — Select this option to remove column headings from the report results.
    • Suppress Report Name — Select this option to remove the report name from the report results.
    • Show Filter Criteria — Select this option to include your selected filter criteria in the report results.
    • Run Offline (will appear in your Report Inbox tomorrow morning) — Select this option for very long reports. Reports that are run offline appear in your Report Inbox the morning after the request.
  5. Click Run Report.

 

Run Report: Hospital Visit History Report
Date Range

Enter the start and end dates for the report, or select a date range from the menu.

Select the format for your report results.

Column Headings
PATIENTID The patient's unique ID
PATIENTNAME The patient's name
DOB The patient's date of birth
SEX The patient's sex
PRIMARYINSURANCE The patient's primary insurance package name and insurance package ID
INSURANCEID The unique ID of the patient's insurance
VISITID The unique ID of the patient's visit
RAZORID The visit's Razor encounter ID
LEVELOFCARE The level of care the patient was in
DEPARTMENT The department the patient was in
HOSPITALROOM The hospital room the patient was in
HOSPITALBED The hospital bed the patient was in
ATTENDINGPROVIDER The name of the patient's attending provider
CHECKINDATE The date and time the patient checked in
DISCHARGEDATE The date and time of the patient's discharge
CYCLEYN Indicates whether the visit has entered the billing cycle (Y or N)
CYCLESTARTDATE The billing cycle's start date
CYCLEENDDATE The billing cycle's end date
REFERRINGPROVIDER The name of the referring provider
