Readmission Report

The Readmission report allows you to track readmission rates at your hospital and monitor trends related to readmission improvement activities.

Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library. Click the Other tab. In the Standard Reports section of the tab, click run next to Readmission Report.

To access reports on the Other tab of the Report Library, you must have the Report: Report Library: Other permission. This permission is included in the following roles:
- Practice Superuser role
- Report Reader role
Note: To activate the Readmission report, use the Activate Reports page.

- Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library.
- Click the Other tab.
- Click run next to Readmission Report in the Standard Reports section of the tab.
The Run Report: Readmission Report page appears. - Discharge date — Enter the start and end dates for the discharges that you want to include in the report, or select a date range from the menu.
Note: You must enter a value in this field to run the Readmission report. - Readmissions Filter (days) — Select the number of days between the index admission and readmission dates: 7, 14, 30, 60, or 90 (the default number of days is 30).
- Level of Care (applies to index and readmission) — To report on readmissions for specific levels of care, click Selected and then select the levels of care from the list.
- Department (applies to index and readmission) — To report on readmissions for specific departments, click Selected and then select the departments from the list.
- Unit — To report on readmissions for specific units, click Selected and then select the units from the list.
- Principal Diagnosis Code — To report on readmissions for specific diagnosis codes, enter ICD-10 diagnosis codes in the fields provided. To use the Diagnosis Lookup tool, type a period (.) in any field.
Note: You can enter an unlimited number of ICD-10 codes. If you fill all eight diagnosis code fields, an additional row of fields appears. - Attending provider — To report on readmissions for specific attending providers, click Selected and then select the providers from the list.
- Length of stay (days) — To report on readmissions based on the length of stay for the patient, enter the minimum and maximum number of days. Leave this field blank for any stay length.
- Discharge disposition — To report on readmissions for specific discharge dispositions, click Selected and then select the discharge dispositions from the list.
- Planned/Unplanned Readmission — By default, the report displays both planned and unplanned readmissions. To report on planned or unplanned readmissions only, select Planned or Unplanned from the menu.
Note: This filter reflects the value of the Elective admission field in the readmission admission order (see To enter an admission order). - Report Format — Select the format for your report results.
- HTML table — Display the report results on your screen.
- Text (tab-delimited) — Export the report results to a .csv file in tab-delimited format.
- Text (comma-delimited) — Export the report results to a .csv file in comma-delimited format.
- Report Options — Select report options.
- Suppress Column Headings — Select this option to remove column headings from the report results.
- Suppress Report Name — Select this option to remove the report name from the report results.
- Show Filter Criteria — Select this option to include your selected filter criteria in the report results.
- Run Offline (will appear in your Report Inbox tomorrow morning) — Select this option for very long reports. Reports that are run offline appear in your Report Inbox the morning after the request.
- Click Run Report.

The Centers for Medicare & Medicaid Services (CMS) automatically calculates readmission data for hospitals using claims data and penalizes hospitals for readmissions that occur within 30 days. Using the Readmission report, you can track readmission rates at your hospital and monitor trends related to readmission improvement activities.
Important: The Readmission report is a supplemental tool to track patients whose index admission and readmission occurred at your hospital. This report does not include patients readmitted to other hospitals and is not intended as an alternative to CMS Hospital Readmission Reduction Program (HRRP) readmission reporting.
