Assign Forms to Appointment Types
This page allows you to map forms on the Patient Portal to specific appointment types so that you can require patients to complete the appropriate form using the Patient Portal before arriving for an appointment. You can also map forms to appointment types when you create or update a form on the Clinical Paper Forms page and the Forms and Documents page.
How do I get to this page?
On the Main
Menu, click Settings 
> Practice Manager. In the left menu, under Practice Links — Miscellaneous, click Appointment Types/Form Mapping
User access and permissions
Your practice must have athenaCommunicator service enabled, and you must have the Practice Set-up or Practice Superuser role to access this page.
To map a form to an appointment type
- Display the Assign Forms to Appointment Types page: On the Main
Menu, click Settings > Practice Manager. In the left menu, under Practice Links — Miscellaneous, click Appointment Types/Form Mapping.
- Click update for the appointment type that you want to map. The Available Forms and Selected Forms boxes appear at the top of the page.
Note: The Available Forms list is populated from the list of Portal-active forms listed on the Forms and Documents page and the Clinical Paper Forms page. (Portal-active forms are indicated by a "Y" in the Portal column on the Forms and Documents page and the Clinical Paper Forms page.) - Click the plus sign (+) next to Client Forms or Clinical Paper Forms to expand the category.
- Check the box for each form that you want to map to this appointment type.
- Click Update.
The forms appear below the patient's appointment on the Patient Portal.
You can ask the patient to download and complete the forms required for the appointment.
