Release Authorizations
This page allows you to create, update, and delete release authorizations and consents, view the audit history of a release authorization or consent, and print a release authorization form. For information about how to create release and authorization forms, see Forms and Documents.
Note: Federal regulations require that authorizations or consents must be obtained from patients before any protected health information can be released to outside parties for reasons other than treatment, payment, healthcare operations, or those specifically allowed by government regulations (for example, law enforcement, public health efforts).
On the Quickview, click Add release authorization (under the Privacy heading)
On the Check-in page, click Add release authorization (under the Privacy heading)
On the Quickview or Check-in page, click Manage Privacy (under the Privacy heading). On the Privacy Information page, click add authorization (the link is on the Release Authorizations and Consents heading)
- Display the Release Authorizations page: On the Quickview, click Add release authorization (under the Privacy heading).
- Form — Select the form to use for the authorization.
- Effective Date — Enter or select the effective date.
- Expiration Date — Enter or select the expiration date.
- Note — Enter any notes about the authorization or consent.
- Click Save and Print Form.
- Present the printed form to the patient or authorized party. After the form is signed, enter or select the date signed in the Date Signed field.
- Click Save. The Quickview page appears. The release authorization or consent is visible on the Privacy Information page, and appears in the Release Authorizations and Consents section on this page.
- Display the Release Authorizations page: On the Quickview, click Add release authorization (under the Privacy heading).
- Locate the authorization/consent that you want to update in the Patient Authorizations list.
- Click the update link. The authorization/consent details appear in the Update Patient Authorization section.
- Update the fields as needed.
- Click Save. The patient's Quickview page appears. The updated release authorization/consent is visible on the Privacy Information page, and appears in the Release Authorizations and Consents sections on this page.
- Display the Release Authorizations page: On the Quickview, click Add release authorization (under the Privacy heading).
- Locate the authorization/consent that you want to print in the Patient Authorizations list.
- Click the update link. The authorization/consent details appear under the Update Patient Authorization heading.
- Click Save and Print Form. The Print Form/Document page appears.
- Select the form that you want to print from the menu.
- Click Print.
- Display the Release Authorizations page: On the Quickview, click Add release authorization (under the Privacy heading).
- Locate the authorization/consent that you want to delete in the Patient Authorizations list.
- Click the delete link. A warning message appears.
- Click OK to delete the authorization/consent, or click
Cancel to keep it. The authorization appears disabled in the list of patient authorizations; an undelete link replaces the delete
link.
Note: To restore a deleted authorization or consent, click the undelete link. The audit history of the authorization/consent is preserved.
