Historical Vaccinations Report
The Historical Vaccinations report displays the immunizations recorded in the Vaccines section of the patient chart and via orders over the specified date range.
Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library. Click the Clinicals tab. In the Standard Reports section of the tab, click run next to Historical Vaccinations Report.
To access reports on the Clinicals tab of the Report Library, you must have the Clinicals user permission and the Report: Report Library: Clinicals permission. The Report: Report Library: Clinicals permission is included in the following roles:
- Practice Superuser role
- Report Reader role
Note: To activate the Historical Vaccinations report, use the Activate Reports page.
- Display the Report Library: On the Main Menu, click Reports. Under General, click Report Library.
- Click the Clinicals tab.
- Click run next to Historical Vaccinations Report in the Standard Reports section of the tab.
The Run Report: Historical Vaccinations Report page appears. - Patient Last Seen Date Range — You can enter the start and end dates to report on patients who were last seen during this date range.
Note: This field is optional. To include in the report vaccinations for patients who do not have a "last seen" date recorded in athenaOne, leave this field blank. - Immunization Date — Enter the start and end immunization dates. Immunizations administered during this date range appear in the report.
Note: This field is required. - Immunization — To report on specific immunizations only, click Selected and then select the immunizations from the menu.
- Patient Min Age Range — To report on patients who are at least a certain age, enter the minimum age for the patient.
Note: The minimum age represents the age of the patient today. - Patient Max Age Range — To report on patients who are no older than a certain age, enter the maximum age for the patient.
Note: The maximum age represents the age of the patient today. - Report Format — Select the format for your report results.
- HTML table — Display the report results on your screen.
- Text (tab-delimited) — Export the report results to a .csv file in tab-delimited format.
- Text (comma-delimited) — Export the report results to a .csv file in comma-delimited format.
- Report Options — Select report options.
- Suppress Column Headings — Select this option to remove column headings from the report results.
- Suppress Report Name — Select this option to remove the report name from the report results.
- Show Filter Criteria — Select this option to include your selected filter criteria in the report results.
- Run Offline (will appear in your Report Inbox tomorrow morning) — Select this option for very long reports. Reports that are run offline appear in your Report Inbox the morning after the request.
- Click Run Report.

Patient ID |
Patient's athenaOne ID. You can click the ID link to display the patient Quickview. |
Immunization |
Full name of the immunization that the patient received. Note: If a vaccine was only partially administered (the full, intended dose could not be administered), this column includes the text "(partially administered)" in front of the immunization name. For more information, see Partially administered vaccines. |
Immunization Date |
Date that the immunization was administered to the patient. |
Data Source |
Source of the vaccine information: Health History or Order. |
Patient Last Seen Date |
Date that the patient was last seen by your organization. |
Patient Age |
Patient's current age. |
Age At Immunization | Patient's age at the time of immunization. |
Patient PCP |
Primary care provider for this patient, as specified in the Usual provider field on the Quickview. |
