The Past Medical History subsection appears in the History section of the patient chart. Use this section to view and record the patient's past medical history.

How do I get to this section?

The Past Medical History subsection appears in the History section of the patient chart. Use this section to view and record the patient's past medical history.

In the patient chart, click the History tab. At the top of the History section, click the Go to menu and select Past Medical History.

Step-by-step instructions

To view and edit past medical history questions in the patient chart

In the patient chart, click the History tab. At the top of the History section, click the Go to menu and select Past Medical History.
If clinical staff answered questions, these questions appear with their answers in this list below the Past Medical History heading.
To view all questions, including questions that have not been answered for the patient, click the view more icon .
Select Yes or No to answer unanswered questions or to edit answered questions. Click the add note icon to add a note.
Click the view less icon to close the item details.

To create a past medical history question

Display the Past Medical History Questions page: On the Main Menu, click Settings > Clinicals. In the left menu, under Practice Links — History/Intake, click Past Medical History Questions.
Click Add new.
QUESTION — Enter a question.
Ordering — Enter a digit in the field to indicate where in the list the question should appear.
Click Save or Save and Add Another.

Features and workflows

You can create past medical history questions and customize this section using the Past Medical History Questions page.

After you configure past medical history questions, you can document Yes/No answers to those questions. For each past medical history question, you can use a notes field to document additional comments.

You can use the Notes field to enter an associated diagnosis, if desired.

To view the audit history, select Audit history in the Patient Actions menu . Click the arrow next to Past Medical History to expand the section. A list of past medical history questions appears in this section. Your practice can manage the list of past medical history questions, including:

Adding questions
Controlling the order in which questions appear
Deleting or updating existing problems
Applying updates to the department level

Problem section versus Past Medical History

The Past Medical History portion of the History section is intended for documentation and review of the patient's medical history at the time of transfer of care into the practice or when patients update this information on the Patient Portal. You should document all active and historical medical problems in the Problems section.
