History Section
athenaOne for Hospitals & Health Systems
You can view a detailed patient history in the subsections of this section. Relevant history subsections appear depending on patient age, sex, and your practice configuration.
Note: Some features of the patient chart are available only in the ambulatory (athenaClinicals) view, and some are available only in the hospital view. Only the ambulatory (athenaClinicals) view includes the Problems chart tab, the Vitals chart tab, and the Visits chart tab. Only the hospital view includes the Diagnoses chart tab, the Flowsheets chart tab, and the Notes tab.
Sub-sections
In the patient chart, click the History chart tab at the left side of the page.
To access the history subsections, click and select the desired section.
In each subsection:
- Click the plus icon to add problems and items to history subsections.
- Click the view more icon to expand the questions in each history subsection.
- Click a problem name to expand the item and view details.
Caution: Use extreme caution when editing checklists and medical history questions. You should edit field names only to fix typos or edit abbreviations. Making substantial changes to content or changing the sequence of checklist items and questions affects all patient charts that include the checklist or question.
With the Multispecialty filtering feature, athenaOne checks the specialty of the rendering provider and user to display only the most relevant information in the History tab of the patient chart and the Intake stage of the encounter. The histories displayed for a particular provider or user based on specialty include:
- Social History
- GYN History
- Past Medical History
- Surgical History
- Family History
You can use the User Preferences page to select your "default" specialty (or set of specialties) so that only information belonging to those specialties is presented to you on the patient chart.
You can use the past Medical History Questions page, the Configure Social History page, and the Configure GYN History page to associate Social History, Past Medical History, and GYN History questions with one or more specialties. You can assign specialties for specific templates or questions.
Patient History questions appear on the History tab of the patient chart and on the Intake stage of the encounter.
In an encounter, history questions appear if the question's specialty association matches the rendering provider's specialty or specialty user preference selections.
Outside an encounter, history questions appear for the logged-in user's specialty or the logged-in user's specialty user preference selections.
Questions with answers but with a specialty association that does not match the user or rendering provider's specialty or rendering provider's user preference are available under "Questions from other specialties" in the expanded view.
Questions that have no answers and are associated with a specialty that does not match the rendering provider's specialty or rendering provider's user preference selections do not appear.
Questions that have no defined specialty always appear.
If no specialty is available from the logged-in user or rendering provider, all questions appear.
