Person Center — Care Team Tab
athenahealth Population Health
The Care Team tab shows current (ACTIVE) care team members as well as previous or historical (INACTIVE) team members. Use the Person Center Care Team tab to view and add members to a patient's care team, to document the full care team for your patients.
On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Care Team tab.
Your organization must use athenahealth Population Health, and you must have the Population Management: Person Center: Quality Care Tab permission, which is included in these roles:
- Population Management: Care Manager role
- Population Management: Care User role

Display the Person Center's Care Team tab for the patient: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed on the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Care Team tab.
- Click the plus icon next to Care Team.
- Select Member — Search for the name of the person you need, and select it from the matching list. (This searches the entire NPI database.)
If the person is not listed, click Add Manual.
Manual entries are saved, so it's easier for you to add them to the care team for subsequent patients.
Enter the member details in the fields provided
- Provider Name — (Appears for manual adds only) Enter the care team member's full name.
- NPI— (Appears for manual adds only) Enter the care team member's NPI, if available.
- Care Team Role — Select a role for this care team member.
- Contact Number — Enter the care team member's contact phone number.
- Email — Enter the care team member's email address.
- Practice Name — Enter the care team member's medical practice name.
- Address — Enter the care team member's medical practice address
- NOTE — Click the note icon to enter a note about this care team member.
- Click Add.
- Care Team Role — Select a role for this care team member.
- Contact Number — Enter the care team member's contact phone number.
- Email — Enter the care team member's email address.
- Practice Name — Enter the care team member's medical practice name.
- Address — Enter the care team member's medical practice address
- NOTE — Click the note icon to enter a note about this care team member.
- Click Add.
athenahealth Population Health automatically adds whoever is listed in the Assigned To field of a program referral to a patient's care team. You can create a new program referral from the Person Center Care Management tab, the Care Manager Patients tab, or the Patient Population page.
- Display the patient's Person Center: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center.
- Click the Care Mgmt tab.
- Next to Programs, click the plus icon .
The Add New Program Referral window appears. - Status — Select Identified.
Note: Only care managers can select the other statuses. See the Care Manager page for more information about status options. - Fill in the other required and optional fields, noting the following:
- Program — You cannot select a program that already has an active status (i.e., Identified, Pending, Scheduled, or Enrolled) for the patient.
- Assigned To — The selected user is added to the patient's Care Team.
- Focus problem(s) — You must display the Focus problem(s) list each time you add another focus problem. As you add focus problems, a tally of the selected focus problems appears in this field (for example, "3 selected"), and the selected focus problems appear highlighted in blue in the list. You can click on a focus problem highlighted in blue to deselect it.
- Status note — Type any notes related to the status update.
- Click Save.
- Display the Person Center Care Team tab: On the Main Menu, enter the patient's name, patient ID, or member ID (insurance number) in the Search Bar. Under NETWORK PATIENTS, select the data type and then click the search icon. Matching patients are listed under the Patients tab. Click the patient name to expand the selection, and then click the patient name in large text to display the patient's Person Center. Click the Care Team tab.
- Click the name of the care team member.
