Self Check-In Questionnaires

This page allows you to configure screening questionnaires for patients to complete during in-office self check-in. If you choose to include screening questionnaires in the self check-in workflow, you map the questionnaires to appointment types using the Self Check-In Questionnaires page.

Note: When using our legacy patient self check-in solution, screening questionnaires are available for in-office patient self check-in only, not for out-of-office self check-in. If you are using Enhanced Self Check-in (released in Fall 2022), patients can complete screening questionnaires during out-of-office self check-in.

 

On the Main Menu, click Settings > Communicator. In the left menu, under Practice Links — Patient Portal, click Self Check-In Questionnaires

 

 

 

To access this page, your organization must use athenaClinicals with athenaCommunicator, and you must have the Communicator Admin user role.

 

 

 

 

To display screening questionnaires to your patients during in-office self check-in, you must map them to your appointment types. Before you create the mappings, make sure that your practice has at least one of the supported screening questionnaires copied to your tablespace (see Screening questionnaires for in-office self check-in).

  1. Display the Self Check-in Questionnaires page: On the Main Menu, click Settings > Communicator. In the left menu, under Practice Links — Patient Portal, click Self Check-In Questionnaires.
  2. Click Add new to create a self check-in questionnaire rule.
    The Add Self Check-In Questionnaires section appears.
  3. Rule Name — Enter a name for the rule.
  4. Appointment Types — Select the appointment types that the rule will apply to.
  5. Questionnaires — Select the questionnaires that you want to map to the selected appointment types.
  6. Click Save.
    The rule appears in the Self Check-in Questionnaires list.
    Note: You can update or delete a rule by clicking the edit or delete link next to the rule name.

 

 

After a patient fills out a screening questionnaire during self check-in, the provider can display the answers during the encounter.

  1. In the Intake checklist or in the Review step of the Exam stage, click Screening.
    athenaOne displays the screening questionnaire score and a notification that the screening questionnaire was completed via online check-in.

    Note: If the patient did not complete the screening questionnaire, the text "Not scored" appears in place of a score. Click the name of the questionnaire to view any patient responses and to complete and score the questionnaire.
  2. Click the questionnaire score to view the details.
    athenaOne displays the completed screening questionnaire.
    Note: Near the top of the completed screening questionnaire, you can select additional screening questionnaires to be completed during Intake.
  3. Review all questions and patient responses.

Update the screening questionnaire as necessary

  1. Finding — Select or change the response to a question and then click Score to recalculate the screening questionnaire score.
  2. Notes — Enter additional details in the Notes field.
  3. Documents — To attach a document to the screening questionnaire, click Select document and select the document that you want to attach.
  4. After you make any changes to the screening questionnaire, click Save.
    Note: athenaOne updates the attribution information at the bottom of the completed questionnaire to reflect the date, time, and user who last modified the questionnaire. (The notification of online check-in disappears from the Screening step of the Intake checklist and the Review step of the Exam stage.)

 

 

If you configured screening questionnaires for patient in-office self check-in, the patient, parent, or caregiver sees the questionnaires after completing the Health History form (see Patient Self Check-In for complete instructions).

Note: For information about the prompt for completing questionnaires, see Who completes screening questionnaires during self check-in?.

 

Following are instructions for the patient, parent, or caregiver to complete screening questionnaires.

Note: Patients can begin in-office self check-in up to 6 hours before the appointment.

  1. Answer the questions in the screening questionnaires.
    Note: You can skip a question if you are unsure of how to answer it.
  2. Click Continue to display the next screening questionnaire, or click Back to go back to the previous screening questionnaire.
    Note: You can see how many screening questionnaires there are and which one you're viewing near the top of the page.

  3. To clear answers for the current questionnaire, click Clear answers near the bottom of the page.

  4. Click Submit after you complete the questionnaire.

 

 

 

Note: Screening questionnaires are available for in-office patient self check-in only, not for out-of-office self check-in.

 

You can use screening questionnaires to assess risk for, or severity of, disorders or conditions such as autism and depression. These questionnaires result in a score and often include guidelines and proposed treatment actions.

 

Patients can complete screening questionnaires (such as the PHQ-9, GAD-7, and M-CHAT) during in-office self check-in. If your practice chooses to include screening questionnaires in the self check-in workflow, you map screening questionnaires to specific appointment types using the Self Check-In Questionnaires page.

 

 

To enable patients to complete screening questionnaires during in-office self check-in, your practice must:

  1. Make sure that the in-office self check-in feature is enabled. If this feature is not enabled, contact the CSC by selecting Support > Create Case or Call in the Main Menu.
  2. Enable at least one screening questionnaire. For information about configuring screening questionnaires, see Screening Questionnaires.
  3. Map screening questionnaires to appointment types on the Self Check-In Questionnaires page.

Note: Any age, gender, and specialty filters that you set up for screening questionnaires on the Screening Questionnaires page apply to in-office self check-in. For example, if the M-CHAT questionnaire has an age filter of 18-36 months and you map the questionnaire to the Well Visit 30 appointment type, the screening questionnaire appears during self check-in only if the child is between 18 and 36 months old.

 

 

 

The following full screening questionnaires can be mapped to appointment types for the in-office self check-in workflow. (For a list of all screening questionnaires available in athenaOne, see Screening Questionnaires.)

Note: These 10 screening questionnaires are the most frequently used screening questionnaires and are currently the only screening questionnaires available for use with the In-Office Self Check-In feature. athenahealth will consider adding additional questionnaires in future releases.

 

These screening questionnaires can be filled out by a parent or caregiver during self check-in:

  • M-CHAT
  • M-CHAT Revised
  • SCARED

These screening questionnaires can be filled out by the patient during self check-in:

  • AUA-SI
  • EPDS
  • GAD-7
  • Geriatric Depression Scale
  • PHQ-2/PHQ-9
  • PSC-17 Youth

The STEADI Fall Risk screening questionnaires can be filled out by the patient or caregiver during self check-in.

 

 

 

During in-office self check-in, screening questionnaires appear after the Health History form is completed (see Patient Self Check-In for complete instructions). The instructions displayed with a screening questionnaire depend on whether the patient or a parent or caregiver is expected to complete the questionnaire, as follows:

  • When a parent or caregiver is completing self check-in and that person should complete the screening questionnaire (for example, the parent of a minor child completes the M-CHAT), athenaOne displays the Screeners page after the Health History form is completed.
  • When a parent or caregiver is completing self check-in but the screening questionnaire (for example, the PHQ-9) must be completed by the patient, athenaOne displays the following instructions:

    The parent or caregiver should hand the device to the patient or click The patient is unable to complete on their own, which ends the self check-in process.
  • When a parent or caregiver is completing self check-in and the screening questionnaire is appropriate for the parent/caregiver or the patient to complete (for example, the STEADI Fall Risk), athenaOne displays the following instructions:

    The parent or caregiver should select the appropriate option to display the screening questionnaire.

 

 

Rule Name

Enter a name for the rule.
Appointment Types Select the appointment types that the rule will apply to.
Questionnaires Select the questionnaires that you want to map to the selected appointment types.
